Information Needed to File a Claim

Completed claim form, itemized receipt, name of service provider, recipient's name, description of service, dates of service, and reimbursement amount. For paper claim forms, log in to your account at www.HealthEquity.com and download a "Lifestyle Benefit Claim Form." *You can submit multiple receipts on the same claim form.

Three Ways to Submit a Claim

  1. Online: Log in to your Participant account at www.HealthEquity.com (select Wageworks if prompted) and file a claim
  2. Mobile App: EZ Receipts: Log in and follow the prompts to submit your claim
  3. Fax/Mail: Complete a paper claim form and return to HealthEquity with all legible receipts for processing either by fax (877.353.9236) or by mail to HealthEquity, PO Box 14053 Lexington, KY 40512

Receiving a Reimbursement

Reimbursements are processed via CMC payroll within 1-2 payroll cycles after claim submission. *Reimbursements are treated as taxable income, included on your W-2, and considered eligible earnings under the 401k plan.

Who is eligible?

All benefits eligible employees, regardless of enrollment status or hire date, are provided this benefit.

Can my Lifestyle Benefit be used for my dependents?

The $500 total maximum contribution can be used for approved eligible expenses for you and/or your eligible dependent(s). Multiple claims can be submitted for yourself and/or your dependent(s) until the total maximum $500 reimbursement has been paid.

Will I receive a Lifestyle Benefit Program debit card?

No, you can only file a claim online, via the app, or by fax or mail.

What happens to my balance at the end of the year?

Use it or lose it! Your balance does not roll over. All funds must be used by Dec. 31 and claims must be filed by March 31 of the following year.

What happens if I terminate employment before filing my claim?

Eligibility ends when an employee terminates. Any unused funds are forfeited.
